Medicaid Waiver
Schedule a Medicaid Waiver appointment with ADRC staff
Make an appointment to get assistance with:
- Home and Community-Based Services Waiver Registrations
- Medicare Questions and Counseling
- Caregiver Counseling and Resources
- Prescription Drug Assistance
- Other (topics that take more than a few minutes)
- These appointments are phone appointments.
- If our call is not answered, the appointment may need to be rescheduled.
- If voicemail is available, and our call is not answered, we will leave a message with instructions.
- Anyone who is registering multiple consumers using one email address (for example, community professionals):
- Please note that if you are using your own email to schedule multiple appointments, please call to schedule an appointment to ensure necessary information for each individual is captured or your client may not receive a call back.
How to use the Calendar Feature, and Schedule with the Correct Program
Step 1: Click Schedule
To begin, click on the schedule link in the upper right corner of the calendar screen. That will open the program selection menu.
Step 2: Select Program
Select the program you are seeking assistance with. For Medicaid, weaver registrations, Medigap and other: ALTSD3
For Medicare Fall open Enrollment – prescription drug and Medicare advantage plans: FOE 24 Scrolling over the blue or green box, a checkbox will appear. Make your selection and click NEXT.
Step 3: Select Available Time
Please Note: Dates that are grayed out have no availability, click on a blue day to make your appointment.
Step 4: Fill out Information
Select your desired time, a small form will open. Fill out the information and click next. On the final screen, review your information and click confirm to make your appointment.
