Care Transition Program

This free program helps residents transition from long-term care facilities back into a community setting. We provide individuals with a clear pathway regarding available choices for long-term service and support options for those who like to return home, or to another residential setting in the community.

The Care Transition Specialists serve as advocates for individuals who require assistance during the transitional process. Our staff works with the individual, the long-term care facility staff, family members, guardians, community service providers, and others to ensure that transitioning individuals are connected to programs and services to help ensure the greatest level of independence possible in a community setting.

  • Do you have a family member in a nursing facility and need assistance getting them home?
  • Are you a resident of a nursing facility who would like to return home or to the community?
  • Are you needing caregiver services or home modifications ?

What kind of help can I get?

  • Assist in providing linkage to community resources such as housing options and obtaining personal documentation.
  • Assist in ensuring that you receive the public benefits you are entitled to receive.
  • Provide education on public assistance programs such as Medicaid and Medicare eligibility and application processes.
