Ombudsman

Residents, their families, friends, and caregivers entrust long-term care and nursing facilities to provide safe environments that meet standards of care and provide a quality of life. If ever they fall short, the Long-Term Care Ombudsman offers an avenue for expressing and resolving concerns.

Care Transition Program

This free program helps residents transition from long-term care facilities back into a community setting. We provide individuals with a clear pathway regarding available choices for long-term service and support options for those who like to return home, or to another residential setting in the community.

Caregiver Resources

A family caregiver is anyone who is an informal provider of in-home or community care for a relative or loved one. You may assist with activities of daily living such as bathing, dressing, eating, and medical tasks. You may run errands, take care of the house, take your loved one to appointments, or coordinate care. Most people, at some point in their lives, become caregivers. There are a variety of supports and services that may be of assistance to you.

New MexiCare

New MexiCare provides financial assistance and training to caregivers who are assisting friends or family members with daily activities due to physical or cognitive limitations.

VDC Veteran Directed Care

Veteran Directed Care (VDC)

VDC is a self-directed that allows Veterans and their families choose the services and supports they need to be able to safely and successfully live at home.

Kinship caregiver Pilot Program

The Kinship Caregiver Pilot Program is now available to residents of Rio Arriba and San Juan Counties. The program is designed to assist kinship caregivers such as grandparents, aunts, uncles, other relatives, and fictive kin who are raising children whose parents are unable to care for them.

Monthly Interactive Training Webinars Sponsored by the New Mexico Aging and Long-Term Services Department. Free Webinar Trainings, earn 2 Continuing Education Credits per session. Trainings are open to professional and family caregivers. CE credits available to social workers and other professions.
