Care Transition Volunteer Program (CTVP)

The new Care Transition Volunteer Program (CTVP) will provide volunteer-based transportation, chore and companionship services to adults 60+ transitioning from long-term care facilities in Taos and Rio Arriba counties.

These services are available for up to 10hrs a week!

To request a program volunteer before you discharge, complete the online application below to request a Care Transition Specialist (CTS) to assist you in getting the services you need.
